Children with multiple disabilities and/or medical complexity ages 3 - 21 who reside in the following counties are eligible:
NY: Westchester, Putnam, or Rockland County or the 5 boroughs of NY.
CT: Fairfield or Litchfield County
You may apply at any time. Approvals occur twice per year in Spring and Fall.
